The initiative
InterACTIVE-HD is designed to improve the quality of life (QoL) for patients in dialysis who live on the Swiss/Italian border, lowering death rates and comorbidity. As its starting point, the project is based upon the exceptional results of the DialysIS project (Interreg 2007-2013), while the enabling factor is the presence of centres geared towards continuous improvement and the provision of increasingly better performing services, assisted by performance-monitoring systems.
Several measures can be taken to address this problem, both clinically and organisationally. Compared to DialysIS, the new system will monitor a larger set of data, and these will be collected automatically. The key objective is to ensure that treatment data are both structured and standardised, allowing the multicentric validation of decision-making support systems, which can then be integrated into the clinical protocols, making it possible to personalise the dialysis prescription and so improve the patient’s QoL. Another aspect that will be considered is home treatment, an ideal solution in cross-border mountain locations, such as those in question. An indicator dashboard will allow operators to monitor performance at the centres. The guidelines, protocols and best practice will all be shared, ensuring continuity of treatment throughout the area, while providing patients with personalised high-quality treatment.
